Team Member Transition Checklist
You will need to have the following documents readily available during the employment transition:
- An AMR earnings statement within the last thirty (30) days (to verify salary)
- Acceptable documents required to complete the U.S. Citizenship and Immigration Services Form I-9
- Most common: U.S. Passport OR Driver’s license AND U. S. Social Security card or Certificate of Birth

- A copy of all applicable EMS National Registry Certifications
- A copy of all applicable Oklahoma EMS license(s)
- Copies of CEU documentation that would support your next refresher certification
- These should be available for download from your current Ninth Brain account
- Direct Deposit information from your bank (Checking or Savings Account)
- EMSA only utilizes Direct Deposit for payroll
